Confidential Information Form (CIF) The Oregon Confidential Information Form (CIF) is a form used by Oregon employers to document employee personal information which must be kept confidential. It is required by the Oregon Bureau of Labor and Industries (BOLD). The CIF is used to protect employee information such as social security numbers, addresses, telephone numbers, and financial information, among other information. It must be signed by both the employer and employee in order for it to be valid. There are two types of Oregon Confidential Information Forms (CIF's): the standard CIF and the enhanced CIF. The standard CIF is used for basic confidential employee information such as name, address, and Social Security Number. The enhanced CIF is used for more sensitive information such as financial information, criminal background checks, and other sensitive information. Both forms must be signed by both the employer and employee in order for it to be valid.
